The town is located 15 km north of the town of Popovo, 48 km northwest of the regional center of Targovishte and about 63 km south of the city of Ruse, located at an altitude of 180 - 200 m above sea level in the northeastern part of the country and occupies the extreme northwestern part of Targovishte region, on the Popovo - Ruse road.
To the south and east it borders with the municipality of Popovo, to the north - with the municipality of Tsar Kaloyan from Razgrad region and to the west - with the municipalities of Dve Mogili and Byala from Ruse region.
The city is located in the eastern part of the Danube hilly plain, in a valley and on its two neighboring hills - Bolets and Ak Tepe. The Ryakata River, a tributary of the Cherni Lom River, flows through its middle. The relief along the river is flat, and in the rest of the part it is strongly folded and cut by numerous valleys.
In the vicinity of Opaka there are several plateaus and numerous karst caves. The town's land borders those of the villages of Krepcha, Ljubljen, Sadina, Gagovo, Palamartsa and Golyamo Gradishte. Asphalt roads connect it with the neighboring villages, except for the village of Sadina and the village of Palamartsa, which are connected by dirt roads.
The territory of the Municipality is 156 sq. km and according to the latest administrative division it includes 6 settlements: the town of Opaka and the villages of Krepcha, Golyamo Gradishte, Gorsko Ablanovo, Garchinovo and Ljubljen, with a total population of 8,589 people. About half of the population lives in Opaka, which is the administrative center of the Municipality.
